What to include
Include your name, the email address associated with your VSEEE account, the type of request and enough detail to identify the information involved. Do not send passwords, payment-card information or patient information.
Verification and response
VSEEE may need to verify your identity and authority before acting on a request. The information requested for verification will depend on the request and the information involved. VSEEE will respond within the period required by applicable law.
Limits and exceptions
Privacy rights vary by jurisdiction. A request may be limited or denied when permitted or required by law, including when information must be retained for security, fraud prevention, legal compliance, accounting, dispute resolution, exercise of legal rights or completion of a requested transaction. VSEEE will explain a denial when required.
Authorized agents and appeals
If applicable law allows an authorized agent to submit a request, VSEEE may require proof of authorization and identity verification. If you believe a request was handled incorrectly, reply to the decision or submit a new request labeled “Privacy appeal.”
